Documentation Requirements
On your first visit to Aldershot Foot Clinic & Mobile Services, for any new service, it is required that you sign a consent form and fill out an intake form.
Please note that payment is expected in full when services are rendered. The services provided are NOT covered by OHIP but may be covered by your extended health insurance provider.
No-Show Policy
To provide efficient services to all patients, Aldershot Foot Clinic & Mobile Services enforces a fee for any missed appointments; We require a minimum of 24 hours' notice for appointment cancellations or rescheduling. Failure to attend a scheduled appointment or provide adequate notice will result in the appointment being charged in full. This ensures that we can provide quality, timely services to all patients as efficiently as possible. If it is your first time utilizing a service, please arrive about 10 minutes early to fill out any necessary forms.
A missed appointment notice and invoice will be issued via email and must be paid prior to rescheduling or booking another appointment.
If you book an appointment within the 24-hour time frame, the policy is in effect immediately.
Please note any fees cannot be waived by any of the staff members, including the front desk. Patients are to email info@aldershotfootclinic.com should any questions or concerns arise.
Lateness Policy
We encourage every patient to arrive on time. For new patients, please arrive 10 minutes early if you have not filled out the new patient intake form prior to your arrival.
We will accommodate you to the best of our abilities. However, if you arrive 10 minutes or more after your scheduled appointment start time, we will need to reschedule your appointment. This ensures that we have sufficient time to carry out the proper treatment and that other patients receive quality care on their scheduled appointment time. The original service cost for the appointment still applies.
No-shows, late arrivals, and cancellations affect not only our foot specialists but our other patients as well. We keep a waiting list for urgent and high-risk cases that would benefit from our services sooner than their scheduled time.
Our policy is similar to many other medical clinics, and we kindly ask all patients to adhere to it. We are aware that there are rare circumstances where 24-hours notice for cancellation cannot be given. On such occasions, we will consider if it is deemed reasonable to make an exception.
Visit Length
The Length of each service can range from 1 hour to 20 minutes. If you have several concerns, please inform the receptionist upon booking the appointment to ensure you have enough time to express your concerns during your visit.
